Can audit records be altered after the fact?
No. Every change to participant records, progress notes, incidents, invoices, and worker credentials is recorded with what it was before and after, and who made it. Notes lock once submitted, and any correction is kept alongside the original with its reason. Audit records cannot be edited or removed from the application, they are write-only at the database layer.
